HIV Navigaton Specialist

Title of Position: HIV Navigation Specialist
Division: LGBTQ Services Department
Status: Full-time/Non-Exempt Job Duties & Responsibilities:\he HIV Navigation Specialist will provide the full range of HIP services, including: outreach, engagement & education, HIV Rapid Testing, linkage/navigation services to PrEP/PEP, and to HIV clinical care, including assistance with treatment adherence, evidence-based behavioral interventions, partner services, and case management.

Essential Functions:

  • Conducts outreach, engages and provides educational components to target population in partnership with East LA Women’s Center Promotoras.
  • Provides rapid HIV testing and confidential risk assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services.
  • Responsible for providing Rapid HIV testing on-site and through outreach into the community, where the target population socializes and/or congregate as needed.
  •  Provides access to biomedical prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.
  • Provide first-time medication drop off to client’s within 5-10 mile radius.
  •  Provides, and ensures access to the screening of STIs.
  •  Provides positive HIV test results to individuals who have tested positive for HIV.
  •  Responsible for providing navigation assistance with HIV clinical care around treatment adherence, partner services, and case management for clients who test positive for HIV. 
  • Collaborate with Program Director and East LA Women’s Center staff to adapt Sister-to-Sister Evidenced Based Intervention for cisgender, heterosexual, Latina women.
  •  Attend and assist ELAWC staff with conducting Sister-to-Sister interventions.
  • Collaborates with project staff in advertising and media promotion of the program.
  •  Maintains communication between inter, and intra-agency services providers for successful coordination of services.
  •  Maintains records of all activities 
  • Attendance at required agency meetings, and external meetings with other local providers.
  •  Assists in documenting program activities and reports outcomes to Department Director along with entering data into the appropriate system.
  •  Perform additional duties as assigned
  • Skills and Abilities:
  • Excellent oral and written communication skills.
  • Knowledge of computer applications and great phone etiquette is a must.
  • Ability to multitask, work well in teams, and be highly detailed orientated.
  • Being respectful and culturally competent is also needed, along with being compassionate and identifying with patient needs and concerns. Must have knowledge of HIV, HIV Treatment/Care, and PrEP/PEP, and ability to educate about HIV, HIV Treatment/Care, and PrEP/PEP.
  •  Ability to complete the following trainings/certifications as needed: HIV 101, Basic 1 HIV Tester/Counselor Certification, PrEP 101, Partner Services, and Motivational Interviewing (to disclose positive HIV results) is also a must.
  • Knowledge of HIPAA and be able to communicate knowledge about biomedical infections (HIV, HCV, and STDs).
  • Knowledge of social services and referrals for patients. Must be comfortable working in a clinical setting with providers and medical personnel.
  • Qualifications for Position:
  • Minimum Education/Training: High School Diploma or GED required.
  • Minimum Experience: At least one year of experience as a Patient Navigator, Case Management, Education or Human Services and great customer service experience is preferred.
  • Certification: Must have active Basic 1 Rapid HIV Testing certification from LA County DHSP or open to attending the training.
  • Previous experience working in sexual health and HIV education, substance use, and outreach highly desired
  • Previous experience working with LGBTQ+ identified individuals highly desired
  •  Applicants fluent in Spanish are strongly encouraged to apply
  • Reports To: Director of LGBTQ Services
  • Travel required or Special Conditions:
  • Must have a reliable automobile for use on the job (mileage to be reimbursed);
  • Subject to a criminal background check prior to employment;
  •  Must possess valid California Driver License;
  •  Must have and maintain State required automobile insurance coverage; and
  • TB, Influenze and pre employment physical clearance, to be renewed every year
  • All COVID Vaccines and Boosters required
  • Work Schedule/Hours: 8:00am – 5:00pm daily with some evenings and weekends required
  • Salary - Hourly Non-Exempt $ 20 to $25, dependent upon experience.
