JOB DESCRIPTION

 

JOB TITLE: Family Nurse Practitioner/ Physician Assistant   

DIVISION: Medical Services                                                     

SUPERVISOR: CMO                                                                 

STATUS: Full Time / Exempt

Via Care Community Health Center

                                     

 

DUTIES AND RESPONSIBILITIES

 

Perform Medical Histories, Physical Exams, Assessment, and Plan of clinic patients based on chief complaint and/or health status and document Health Care Maintenance status as per established protocols

  • Carries out mission, vision and values established by the CEO/Chief Medical Officer and Board of Directors;
  • Adept at Women's health, OB, and pediatrics. Must be comfortable seeing all life cycles.
  • Perform and or assist in the performance of laboratory and screening procedures delegated by the supervising physician;
  • Perform or assist in the performance of the therapeutic procedure delegated by the supervising physician;
  • Recognize and evaluate situations, which call for immediate attention of the physician;
  • Instruct and counsel patients regarding matter pertaining to their physical and mental health;
  • Facilitate patient referrals to the appropriate health facilities, agencies, and resources of the community;
  • Review treatment and therapy plans, order routine diagnostic laboratory tests and procedures, and routine diagnostic radiological services; and
  • Performs other duties as assigned by supervisor.

Qualifications

 

The requirements listed below are representative of the knowledge, skills, and/or abilities required to perform the job successfully.

 

Education/ Experience / Skills

  • Required Valid State of California Medical License (PA/FNP);
  • Valid DEA License;
  • Valid Certificate of Professional Liability Insurance; and
  • Required: English / Spanish Bilingual.
  • Salary $125,000 to $150,000, dependent upon experience.

Special Requirements

 

  • Must have a reliable automobile for use on the job (mileage to be reimbursed);  
  • Subject to a criminal background check prior to employment; 
  • Valid California Driver License; 
    • Must have and maintain State required automobile insurance coverage; and 
    • TB clearance, to be renewed every year, and proof of MMR and TDAP vaccinations and current Influenza Vaccination  COVID vaccine & Subsequent Boosters

Must be able to talk and hear well.  Good vision is imperative.

 

The following lists physical demands an employee may perform on an occasional basis: lifting an average weight of 20 lbs., crouch/knee/stoop/crawl and reach with hands and arms.
